At any time in your life, you may be unable to communicate your health care preferences due to injury or illness. Advance care planning encourages the understanding of possible health care choices, talking about decisions with loved ones and doctors, and having plans in writing if needed, It is a gift you can give to yourself and your loved ones. Having a plan will make it easier for you, your doctor, and your loved ones to ensure your wishes are honored.
This free workshop will help identify health care choices and suggest ways to begin communicating them to family and health care providers. It will also explain the use of advance directive documents: health care power of attorney and living will.
Forms, resources, and notary services will be provided for those wishing to execute their advance are planning documents.
